Who Do We Serve?
The Cooperative Health behavioral health team specializes in treating patients of all ages. Our providers accept both insured and uninsured patients. Ask your primary care physician for a referral and call now to make an appointment, or you can use our online form.
If you or someone you know is experiencing a mental health crisis, having thoughts of suicide, or is in immediate danger, please do not use this contact form. Instead, call 911 for emergencies or contact the Suicide & Crisis Lifeline by calling or texting 988 — free, confidential support is available 24 hours a day, 7 days a week. You are not alone, and trained crisis counselors are ready to help.
If you are not in crisis, contact the nearest Cooperative Health office for medical care or the office you normally visit to reschedule an appointment.
After you submit your appointment request, our scheduler will respond within 48 hours. Once you have made your initial appointment, we recommend that you complete the new patient.
